New medication policy for Mesa Public Schools that will begin with our next school year 2016/2017. There is a new "Medication Consent Form". It requires the physician to complete the bottom portion if your child will be taking the medication 5 or more days in a row.

MEDICATIONS FOR ALL STUDENTS ARE REQUIRED TO BE DROPPED OFF BY THE PARENT OR RESPONSIBLE ADULT EVEN WHEN MONTHLY REFILLS ARE NEEDED.

Please remember to review your child's immunizations before the next school year. Student's age 11 years are required to have a TDAP (tetanus, diptheria and pertussis) and MCV(meningitis) vaccinations.

If your child has a medical condition that requires an emergency plan such as a severe allergy or seizure history we will need a new plan completed at the beginning of the school year. We can set up meetings with teachers as needed. Feel free to call the nurse with questions.  480-472-8334.
