Reminder- if your child has a fever and symptoms of illness please keep them home until they are fever free for at least 24 hours without the need for medication. Is my child well enough to be at school today?

"Medication Consent Form". It requires the physician to complete the bottom portion if your child will be taking the medication 5 or more days in a row at school.

Medications are required to be dropped of at school by a parent or responsible adult.

Please remember to review your child's immunizations before the next school year. Student's age 11 years are required to have a TDAP (tetanus, diptheria and pertussis) and MCV(meningitis) vaccination.

If your child has a medical condition that requires an emergency plan such as a severe allergy or seizure history we will need a new plan completed at the beginning of the school year. We can set up meetings with teachers as needed. Feel free to call the nurse with questions.  480-472-8334.
