GETTING AN INSTRUMENT REPAIRED
Follow all steps below to ensure more timely service.

Non-Emergency Repairs

  1. Complete a Repair Work Order.  You must use Chrome and be logged into your school e-mail for this to work properly.
  2. Fill out an Equipment Transfer Form with all the pertinent information.  Check the box on the form to TRANSFER the instrument.
  3. E-mail a copy of the Equipment Transfer Form to Spencer at instrumentrepair@mpsaz.org prior to the pick-up date.
  4. Attach a repair tag to the instrument case.  Please be specific on the tag with the repairs that need to be done to the instrument.  Please do not send an instrument in without any information regarding what isn't working.  If you need more tags, contact Spencer at instrumentrepair@mpsaz.org or (480) 472-1056.
  5. Bring instrument(s) to the front office of the school or to your designated MDC pick-up area.  Place a copy of the Equipment Transfer Form with the instrument(s).  Whenever possible, please wait until the working day before your designated school pick-up day to bring the instrument to the pick-up area.
  6. When the instrument has been repaired, Spencer will complete a transfer form to send it back to your school.  It will be delivered on the next weekly pick-up day.

Emergency Repairs Only

Examples of emergencies include having an audition or a performance in the next few days.  Just because an instrument is being used by a student does not constitute an emergency.  Though we would like to know if the instrument is currently being used, please use the emergency repair option only in true emergencies.

1.  Call Spencer at (480) 472-1056 to arrange a time to hand-deliver the instrument to the shop.

2.  Complete a Repair Work Order.  You must use Chrome and be logged into your school e-mail for this to work properly.

3.  Attach a repair tag to the instrument case.  Please be specific on the tag with the repairs that need to be done to the instrument.  Please do not bring an instrument in without any information regarding what isn't working.

4.  Personally deliver the instrument to the shop at the time agreed to when you called Spencer in step number 1.  Every effort will be made to accommodate the repair of this instrument in a timely manner.  Please understand that we may not always be able to fix things "on-the-spot."  We will do our best.  DO NOT SEND A PARENT OR STUDENT TO THE REPAIR FACILITY.  Teachers only please.
