Life Insurance Coverage
Remember that the District provides Basic Life Insurance coverage for you at no cost to you. Please enter the following information for life insurance even if you have previously provided this information on a paper form:
Beneficiaries for both the Basic Life Insurance coverage that the District provides and for any Supplemental Life Insurance coverage you have on yourself.
If you have supplemental life insurance coverage on your spouse, please make certain to add your spouse's information into the system for this coverage as the cost cannot be calculated without his/her date of birth.
If you experience difficulty in the enrollment system in the life insurance section, there may be some missing information for one of your life insurance plans. Please provide all information requested on each page before attempting to advance to the next page. Also make certain you are in the Open Enrollment section rather than the Current Benefits section.
Benefits Deductions for 2011-2012
If you have a 12 month contract, benefits deductions will be taken from 24 pay periods, beginning with the pay check you receive on October 6, 2011. If you have a contract for a period less than 12 months, deductions are taken from 18 pay periods beginning with the pay check you receive on October 6, 2011. The pay check deduction amounts shown in the system are calculated based on your contract.
Medical Plan Review Meetings
If you have not attended one of the meetings to review the medical plans, today is the last day to attend. Please plan to attend one of the meetings below for information on our three medical plans and how to determine the best option for you and your family.
