IMMUNIZATION INFORMATION

Arizona State has changed the immunization requirements for the 07/08 school year.  All students are required to have the tdap immunization every 5 years.  Students also attending in 08/09 are required to have the meningitis vaccine.  

All 9th Graders need to be compliant in order to register and recieve schedules from the high school.  In an attempt to get all Stapley 9th graders up to date before High School we are offering several free immunization clinics to be done at school.  If you would like to have your childs immunizations given at school simply sign the green sheet that will be sent home with all 9th graders and return it to the school nurse.  After your green sheet is returned the nurse will contact you regarding the immunizations that your student is in need of.  The majority of all students need immunizations 426 out of 466 students in the 9th grade are in need of immunizations.  You may also go to your own physician to receive the needed immunizations and turn in the documented proof  to the nurses office.

Stapley Jr. High will be having a free clinic in September if interested pick up a form at the Stapley Nurses Office.
