Unimerica
District provided life insurance is offered to employees in the following amounts:
Full Time Certified: One times annual salary not to exceed $50,000
1/2 to 3/5 Certified: $20,000
Classified: $20,000 minimum, one times annual salary not to exceed $50,000
Dependents: Spouse - $2,000; Children - Up to $1,000 depending on age
     Must be enrolled as a dependent under District Health Care.
Group Life Accidental Death and Dismemberment Certificate of Coverage
To make changes to your beneficiary designation, you must submit a completed change form to the Benefits Office. Click HERE for the Unimerica basic and supplemental life enrollment kit.

Coordination of Survivor Benefits

Employee Benefits coordinates survivor benefits for employees, and dependents of Mesa Public Schools. The coordination of benefits may include unpaid wages at the time of death, unused annual and sick leave, insurance, retirement, and other related benefits. Departments and/or families should notify Employee Benefits when a death has occurred at (480) 472-7222.
