The Mesa Early Learning Preschool (MELP) Program serves
four-year-old
children from qualifying families.
To enter in the 2008/2009 school year, children must turn 4 by August 31, 2008. Families who attend for free must reside within the Mesa School District boundaries and meet the following income guidelines:

The following information is required for registration:
(Immunizations must be current to register)

A copy of your child's birth certificate

Proof of up-to-date immunizations: DPT-4; Polio-3; MMR-1 (on or after 1st birthday);
Hep B-3; Hep A-2 (6 months apart); HIB-1 (on or after after 15 months)
Chickenpox (Varicella)

Proof of residence (utility bill))

Verification of monthly income (2-3 most recent pay stubs, child support, self-employment, disbility, unemployment or cash/food stamp assistance award letter. If you are self employed or your income is unstable, provide your 2007 adjusted gross wages reported to the IRS on your 1040 or 1040A form.
