Welcome to the Nurse's Office!
Hawthorne Elementary has medical
coverage with Chris Sabo, a Registered Nurse and Glenna Gillespie, our Health
Assistant.
We are available to assist students with illness and injury related problems.
In addition to these primary services, we conduct health screenings, assist
students and families with referrals for medical services, follow-up, and act
as a health education resource person for students and staff. The goal of Health
Services is to remove barriers to health and safety of students, enabling them
to achieve their academic goals at Hawthorne Elementary.
ILLNESS/EMERGENCY
School is no place for a sick child. Please do not send your
child to school if he/she has had diarrhea, vomiting, a fever, rash,
deep cough, or a communicable disease such as chicken pox, strep
throat, pinkeye, etc. in the last 24 hours. You will be notified if
your child becomes ill enough to be removed from school, sustains
more than a minor injury, or has an emergency. It is parents'
responsibility to make arrangements to pick up an ill or injured
child as promptly as possible. We suggest that you have someone
listed on your information sheet in case you are not available or cannot
leave work.

CELL, OR
EMERGENCY PHONE NUMBERS FOR YOUR CHILDS STUDENT INFORMATION.

DISTRICT MEDICATION POLICY
Under certain unusual circumstances when it is necessary for a
child to take medication during school hours, the following
requirements must be met:
1. Prescription medication must be in the original container with a non-expired prescription label and directions that include specific dose and time to be given.
2. Over-the-counter medications must be in the original container with directions.
3. Appropriate dosage for age as stated on the label will be dispensed.
4. A written permission medication consent form must be signed by the parent/guardian to administer medication to the child.
5. Dosage, time, dates to be given and name of medication must be stated on the written permission.
6. We recommend that all medication be brought to and picked up from school by a parent/guardian or another responsible adult.
7. Inhalers cannot be kept on the child's person without a written doctor's orders.
8. It is recommended that when you get a prescription filled to be given at school that you have the pharmacists label two bottles and send the appropriate one to school. (This is usually free.) This is important with liquid medicines that have to be refrigerated like antibiotics. Medications that are given 3 times a day need to be given before and after school.
9. Medications brought to school and not meeting necessary requirements will not be dispensed by the nurse and will be locked in the office until a parent/guardian verifies the medication, signs the proper form or takes it home.
EMERGENCY MEDICAL
CARD INFORMATION
These cards
are used to contact a child's parent in case of illness or injury.
They must be completed thoroughly, returned immediately and kept up
to date. It is imperative that you notify the nurse if your child has
a medical condition such as diabetes, seizure disorder, asthma, heart
disease, allergies (food and environmental), a physical or mental
disability, etc. This will help us and the school staff better serve
your child.
IMMUNIZATIONS
Arizona State Law ARS 15-803 states that children must have
immunization (shot) information on file at school. It is the
responsibility of the parent/guardian to furnish the school with a
copy of the official immunization record which includes the month,
day, and year of each immunization. If the student's immunizations
are not complete or up to date, the student may be excluded from
attendance until the needed immunization is received.
Physically and emotionally healthy students are better able to take advantage of their learning opportunities. The Health Office at Hawthorne Elementary is available to students and parents to provide the services that will contribute to that philosphy. We encourage parents to provide their children with rest, food, and an emotionally stable environment at home that ensures their readiness to participate in the learning process.
