Registration

Registration of new students to Mesa Schools will require the following:

Official birth certificate Record of up-to-date immunizations required by AZ State Law Proof of residency

Students transferring in from another Mesa Elementary School will need to complete a New Student Data form and provide proof of residency within the Ishikawa School boundaries prior to assignment to a classroom.

Immunization

DPT -- You will need four doses if the last dose was received on/after the fourth birthday. A fifth dose is needed if the above shots were received before the fourth birthday.

Polio (OPV, IPV) -- Three doses is needed if the last dose was received on/after the fourth birthday. Fourth dose is needed if the above shots were received before the fourth birthday.

MMR -- One dose is needed if received on/after the first birthday. If dose was received before the first birthday, a second dose is needed.

Hib -- Not needed after the age of five.

Hepatitis B -- Only required for ALL Kindergarten and First Grade. Three doses with the time interval: first two doses one month apart; then dose three within five months.

MMR -- Second dose is needed for all Kindergarten and first graders.
