Health Office
Madison Elementary's School Nurse, Mrs. Snodderly, or Mrs. Becker, the Health Assistant, staff our Health Office during school hours. They are available to assist students with illness and injury related problems. In addition to these primary services, the nurse conducts health screenings and provides health education for students. The goal of Health Services is to remove barriers to the health and safety of students, enabling them to achieve their academic goals at school.
Illness/Emergency: School is no place for a sick child. Please do not send your child to school if he/she has had a fever, rash, diarrhea, vomiting, deep cough, or a communicable disease such as chickenpox, strep throat, pink-eye, etc. in the last 24 hours. You will be notified if your child becomes ill enough to be removed from school, sustains more than a minor injury, or has an emergency. It is the parent's responsibility to make arrangements to pick up an ill or injured child as promptly as possible. We suggest that you have someone listed on your Required Emergency and Registration Information form in case you are not available or cannot leave work.
Emergency Medical Information: It is imperative that you notify the nurse if your child has any type of medical condition or takes daily medications. This will help all the school staff better serve your child. This information should be placed on the emergency section of the Required Emergency and Registration Information form also.
Immunizations: ARIZONA STATE LAW ARS 15-803 states that children must have immunization (shot) information on file at school. It is the responsibility of the parent/guardian to furnish the school with that information and initiate having the child receive any needed immunization before they are allowed to attend school. If your child receives immunizations during the school year, please send a copy of the updated record to the Health Office.
