Enrolling
Your Child
To register a
student in a Mesa school for the first time,
you'll need a certified copy of the student's
birth certificate, current immunization record
and proof of residency.
By school board
policy, most children enrolling in kindergarten
will reach the age of 5 by August 31. However,
parents of children reaching the age of 5 between
September 1 and December 31 may request enrollment
in kindergarten if they believe it is in their
child's best interest. Children will be enrolled
where space is available.
Arizona law requires
documentary proof of immunity against certain
childhood diseases for students entering Arizona
schools for the first time. With some exceptions,
students subject to this requirement who lack
documentary proof may enroll but not attend school
until proof is provided. If your child falls
into this category, more detailed information
is available at the school.
Immunizations are
available from family physicians or pediatricians.

available.
Parents will
be asked to fill out a registration form and
an emergency medical referral card.
Proof of residence
must also be shown. Parents may provide one
of the following:
- Rent receipt
- Utility invoice
- Motor vehicle registration
- Housing contract, lease agreement
- State income tax return
- Bank account, pay check, etc.
- Employer letter on letterhead
