Student Registration Information

Before you register a new student, please be aware that you will need the following information;

If a student is new to the Mesa Public School District, you will need

  1. Official birth certificate (not a copy and not a hospital certificate)*
  2. Immunization records
  3. Proof of address
  4. Name and address of last school attended (if applicable)

    *If your child was born in the state of Arizona, and if you don't have a legal copy of the birth certificate the following is the information that you need to obtain one;

REGISTRATION FOR CURRENT MESA PUBLIC SCHOOL STUDENTS

If your child is currently enrolled within the Mesa Public School District you will need to submit the following in order to transfer the student to O'Connor School

  1. Proof of address in O'Connor attendance area
  2. Fill out a student data form
