Q. How do I change my Federal taxes? TOP |
A. Complete the Federal Tax Change form (W4) and send it to Payroll. The form is available on the internet in Documents and in the Payroll Office. |

A. Complete the Arizona State Tax form (A-4) and send it to Payroll. The form is available on the internet in Documents and in the Payroll Office. |

Q. What should I do if I lost my payroll check? TOP |
A. Come down to the Payroll office to complete the appropriate paperwork and have it notarized. A Notary will be available in either Payroll or Human Resources free of charge. You will need a state ID in order to get the paperwork notarized. In the event that the payroll check belongs to a minor without a state ID, a guardian must be present with there state ID. There is a 48 hour waiting period before a replacement check is issued. |

Q. What should I do if I do not receive my check?TOP |
A. Contact your payroll specialist to confirm a check was issued. If the check was issued, we will research to see where the check was sent. If it is determined to be lost in the mail, it is a 10 day waiting period beginning with the date it was mailed before we can stop payment. You will need to complete some paperwork in Payroll and have it notarized before a stop payment can be placed. There is a 48 hour waiting period before the replacement check is issued. |

A. To start, first you need to be established with a company from our “List of Participating Investment Companies” before establishing a payroll deduction. Then complete the 403(b) Salary Reduction Agreement form to begin, change, or stop your deduction and submit it to Employee Benefits for Authorization. A. The Mesa Unified School District holds its annual U. S. Savings Bond promotion during August and September each year. You have the opportunity to enroll in our payroll deduction program for U.S. Savings Bonds, or increase the amount of savings if you're presently enrolled. Deductions begin in October for 16 pay periods. To stop your deduction, contact Adrian Lopez in Payroll at 480-472-0445. |

A. Employees of Mesa Public Schools may donate through A Dollar Makes a Difference -- the foundation's payroll deduction plan. Mesa Public Schools employees may donate a dollar or more to the Mesa Foundation for Educational Excellence through payroll deduction. Tax-deductible contributions benefit the students and staff of MPS. |
