School is no place for a sick child. Please do not send your child to school if he\she has diarrhea, vomiting, a fever, rash, deep cough, or a communicable disease such as chicken pox, strep throat, pink eye, etc. If you are unsure of your child's symptoms, please call the school nurse before sending him\her to school. You will be notified if your child becomes ill enough to be removed from school, sustains more than a minor injury, or has an emergency. It is the parent's responsibility to make arrangements to pick up an ill or injured child as promptly as possible.
Children are not permitted to walk home or ride the bus when ill or injured per district policy. Please notify the nurse of any home/work/emergency numbers that change on your child's emergency card.

      DTP-      You will need four doses if the last dose was received on/after the fourth birthday. A fifth dose is needed if the above shots were received before the fourth birthday.
 
      Polio-   Three dose are needed if the last dose was received on/after the fourth birthday. Fourth dose is needed if the above shots were received before the fourth birthday.
 
      MMR-  K-2 Two dose are needed. First dose must be received on/after the first birthday. If one dose was received before first birthday, a third dose is needed.
               4-6th 1 MMR needed
      HIB-  Not needed after age 5
Varicella - K-2 - 1 dose or history of disease.
Hepatitis B Three doses with the time interval: first two doses one month apart; then dose three within 5 months.
