Mail-In Registration

Everything needed to complete the registration process for continuing Poston students will be mailed to your home on July 7, 2008. Each Personalized Registration Packet will contain the following forms with instructions that are easy to follow. We encourage you to complete and to return your completed paperwork with your payment as soon as possible, with it postmarked no later than July 25, 2008.

Your packet should contain the following:
Required Emergency & Information Form: The pre-printed information on this form will need to be checked for accuracy and signed by a parent or guardian. If the IMMUNIZATION NOTICE section indicates REQUIRED, you must provide the nurse with proof that immunizations are current before your student will receive his/her class schedule. By signing the Required Emergency & Information Form and including proof that immunizations are current when returning your completed registration forms, your class schedule will be mailed back to you with your Bookstore Receipt, ID Card, etc. If you do not sign this form and provide proof that immunizations are current (if required), your class schedule will be held until you provide proof and/or sign the form. At that time, you will receive your class schedule.

Fee Explanation Sheet: When Mesa junior and senior high school students register for school each fall, some non-required fee situations and the required A-Hour or Overload class fees may apply. The information on this form will explain the different options available to you. *Please note that full payment for Poston A-Hour classes or Mountain View Overload classes (both semesters) must be paid when you return your completed registration paperwork, even if you pay no other fees.

Registration Fee Sheet: This form lists all of the items that are available for you to purchase. At the bottom of this form is a list of the classes your student will be taking next year. You must cross-reference the required and voluntary material donations that apply to the classes listed to determined what you will include in the payment you send back with your completed paperwork.

Yearbook Information: Please keep in mind that only those students who pay for a yearbook during Registration or at the Bookstore through December 1, 2008 will be guaranteed receiving one at the end of the school year. The cost of a yearbook is $25 with an ID card only and for $23 with the purchase of an activity card. If you miss the deadline, we may have a very limited number of extra yearbooks available the day after pre-sold yearbooks are distributed in May. Any extra yearbooks will be sold on a first-come, first-served basis. The cost at that time will be $30 for everyone.

End-Of-The-Year DVD Information: The End-Of-The-Year DVD set captures many of the great moments from the year. The set includes 7th, 8th and 9th grade sports, musical events, our weekly videos, special events, campus life, video clips from world events and a teacher section. Don't miss out on your Year in Review: Memories for a Lifetime! The DVD set can be purchased for $12. The cost at the end of the school year will be $15.

Internet Access Agreement: All Mesa Public School students will have limited Internet access. A copy of the MPS District Electronic Information Services policy is available upon request. If you DO NOT want your student to have access to the Internet, you must come into the office and sign the Opt-Out form that will authorize us to deny your student access to the Internet.

Mesa Public School Absence Reporting Policy: Each parent/guardian should be aware of the guidelines for reporting a student's absence from school. The Absence Policy, which requires a parent/guardian signature, is located on the back of the Required Emergency & Registration Information Form.

Cafeteria Meal Pre-Payment and Cafeteria Meal Assistance Program: You will have the opportunity to pre-pay for meals. These forms explain the options available to you.

Bus Information: You will receive the Poston Junior High School Bus schedule Information Sheet with a Bus Route Map and guidelines to help you to determine if your student is eligible to ride a bus. Students who are attending Poston on Boundary Exceptions must provide their own transportation.

Poston Self-Addressed Envelope: We will provide you with a return envelope. All you need to do is complete the packet and mail it to us. Your receipt, student's ID Card, class schedule and locker information will be mailed back to you. It's just that easy!
