IRS Form W-9

An IRS W-9 form is required from all firms prior to the issuance of a District Purchase Order.  It is not necessary to have this form completed and on file in order to receive bid opportunities.  The completed form is only required prior to the issuance of a Purchase Order.  Visit our  "Vendor Set Up" page to submit your W-9 information.
