Athletic Eligibility
Please print ALL eligibility forms listed below. Forms need to be completed in blue or black ink (not pencil). All forms must be submitted to the athletic department in the administrative office. Please review all forms to make sure information is correct and that parent/guardian and student signatures and dates are provided.
Incomplete physical packets are not accepted. Student eligibility must be approved prior to the start of tryouts.
Per AIA rules, physicals must be dated AFTER March 1, 2013 for the 2013-2014 school year. Please print out the Brainbook Concussion Certificate and submit with physical packet.
Pay to Play: Payments MUST be made in the Red Mtn. Bookstore, or via credit card by calling 480.472.8023. A receipt will be issued for all payments and may be used toward an ECA donation.
