Arizona state law requires that children must have immunization (shot) information or an immunization exemption on file at school. It is the responsibility of the parent/guardian to furnish the school with this information.
You will be asked to provide a copy of your child's immunization record upon enrollment. You will be notified if your child’s immunization record is or becomes inadequate. Please contact the Health Office if you have questions about any immunization notices you may receive.
Parents of Fourth, Fifth, & Sixth Graders- Please note:
All students must have a Tdap vaccine (for tetanus, diphtheria, and pertussis) and a Meningitis vaccine once they turn eleven years old. Please plan to take your child to his/her health care provider or an immunization clinic once he/she turns eleven.
