Information for 2012-2013 Sports Physicals
All Skyline Athletes for the 2012 - 2013 school year can have their required sports physical conducted right here at school! On Thursday, May 3 in our gym lobby, team doctor, Dr. Green and physicians from his medical office, will be here from 3:30 - 5:30 pm to conduct physicals. The sports physical will cost only $15 with all proceeds going to Skyline Sports Medicine. All interested athletes need to stop by the Skyline Bookstore to pay $15 before receiving a sports physical. Please pick up a sports physical packet from the Athletics secretary, Mrs. Rudzinski and have your parent complete one form. You can also print the forms below to fill out. Bring this physical packet, along with a receipt from the Bookstore indicating you've made your $15 payment, to the gym lobby between 3:30 - 5:30 pm on May 3rd to receive your sports physical.
