SMITH REGISTRATION INFORMATION
New Student Registration
Welcome to the Smith Sidewinder Family! If you have just moved into our boundaries, please complete the MPS Registration paperwork (click the Red button above) and mail, fax, email or bring them in to our office.
Along with the enrollment packet please make sure to provide us with the following documents:
- Original Birth Certificate
- Proof of Immunizations
- Proof of Residency (i.e. utility or phone bill, lease or mortgage documents, vehicle registration, etc.)
- Photo ID of Enrolling Parent/Guardian
Once done, please have your child complete the class/elective choices (form available in our front office).

We offer Open Enrollment for families who reside outside of Smith Jr High's boundaries, but wish to attend our school. Please follow the link (Blue button) above for more information and forms to complete.
Acceptance is on a year-by-year basis and students need to be in good standing with their current school. As noted on the MPS Open Enrollment Form, parents and students must agree that students will honor all school rules... including standards for academic effort, conduct and attendance.
Applications are reviewed in the order they are received. Receipt of your application does not guarantee acceptance to Smith Junior High. You will be contacted upon the approval status of your child. For any application submitted before March 1, the student will be notified of the acceptance decision by April 1. If accepted, the school requests confirmation within two weeks of notification that the student will attend. The names of applicants who are not accepted will be placed on a waiting list.
