Frequently Asked Questions 1. What information do I need to register my child?
Register a student top Please bring the following information with you when you register your child. 1.Proof of address: Driver's license, a bill (SRP, Qwest, Water, etc.), lease or rental agreement or a bank statement. 2. Birth Certificate 3. Immunization Records (Official records or copies) 4. Complete name and address of last school attended. School Hours top
Registration for Kindergarten is the last week in January. Please check the marquee, the weekly newsletter, or the calendar on our website for updates. Child absent from school top Please notify the Sousa office at 472-8890 as soon as possible, providing the student's name, teacher's name and the reason for the absence. If your child's absence will be extended, please notify the office. Late to school/ Pick up early top In case of an emergency, we need to know who is in the building at all times. Therefore, if you are bringing a student in late, sign them in at the office. The student will be given a late slip to take to class. If you are picking a student up before the end of the day, come to the office to sign him/her out, and we will call for him/her. Please do not go to his/her classroom. Other Ways to Contribute top Have you heard about the Arizona School Tax Credit? Basically you can redirect $250 of your Arizona taxes to Sousa. You can even select which extracurricular activities you would like your money to support, things like the computer lab, 5th grade science camp, field trips, band, orchestra and the after-school programs. The office can provide you with the form for the tax credit donation. You could let us know if you have any special talents or interests that you'd be willing to share with us. You could support your child's teacher by offering to bring in special materials when needed. You could contact the Sousa PTO or Sousa After-School Program to see if you could be of some assistance to them in acquiring donations from local business. Most importantly, you can support your child by insuring that they arrive to school on time, have a good breakfast, have time to study after school and have a good night's rest. Lost and Found top
If your child loses an article of clothing, school materials, a lunch box or other possessions, please check the lost and found box located in the Multi-Purpose room. We request that personal possessions be labeled with a child's name so that we may easily locate the owner. Unclaimed items are donated to local charities periodically. Children should not bring expensive or treasured personal possessions to school, since we cannot be responsible for lost or stolen items.
