Q : Where should I send the white slip (request for file)?
A : The white slip is to be sent to the last school attended if the student attended the previous school year. If you receive a duplicate copy, request another copy of the birth certificate for file.
Q : I have a Homebound student, where can I get information on attendance?
A : Call the Homebound registrar listed under Guidance and Counseling at X87551 or check the Guidance and Counseling website for more information.
Q : Where can I find information as to who my contact person is at Student Records?
A : There is a listing on the Student Records website, under the Employees Only webpage - School Listing by Region.
In order to serve you better, please address all correspondence to Student Records-Attention:_______ on the outside of all envelopes, including white slips used for records requests.
Q : Does Student Records still have files from the 2008-2009 school year?
A : No, files from 2008-2009 and before have already been destroyed. A duplicate copy of the Perm Card will be sent and you may need to recreate it.
Q : Can files with out of district requests be sent in with my withdrawals or no-shows?
A : No, files with requests are to be sent in separately from your withdrawals or no-shows and with their own transfer sheet.
Q : Can I shred old immunization records when I get an updated one?
A : Yes, as long as all the information has been transferred from the old record to the new updated record.
