Note: The following is a shortened version of the some of the registration information (registration packet) that was mailed early in July to Taylor families. 


Please refer to the actual Registration Packet for complete information.

1. Check the IMMUNIZATION NOTICE at the top of the  Required Emergency & Registration Information form. If the IMMUNIZATION NOTICE indicates REQUIRED you must provide the nurse with PROOF that immunizations are current when returning the registration form or by the first day of school. If your child has had the chicken pox vaccination (VZV), please send a copy of the immunization record to the school nurse.

6. Cafeteria: For your convenience you can pre-pay for school breakfasts and lunches. Refer to the PEACH "Meal Program form" to determine the cost of meals. If you want to take advantage of this option, you must complete the Cafeteria Meal Prepayment Form with all the requested information to ensure that your student's account is properly credited. The Registration Packet also includes the MPS Cafeteria Meal Assistance Program information. To complete this form, just follow the instructions printed on the form and return the form with your registration packet.

2. Check all information on the Required Emergency & Registration Information form.  Make any changes to phone numbers and addresses. Be sure that all phone numbers are current. Fill n all the spaces in the "Emergency or student being sent home" section. Complete all questions regarding health problems, medications & recent surgery, accidents or illnesses. Indicate whether you give permission for the nurse to give your child acetaminophen. A signed consent form must be turned into the nurse's office for all other medication (including inhalers) taken during school hours. Forms are available in the nurse's office.

7. Registration Fees: All fees are OPTIONAL with the exception of junior high "A" period classes. Refer to EXPLANATION OF OPTIONAL FEES. Optional fees may include activity card (which saves you money throughout the year), yearbook or memory book. material usage fee for specific classes, and money which can be placed in your student's breakfast/lunch account. Please complete the  registration Fee Schedule carefully and include your payment when you return the registration packet. Payment can be made by check or money order (with your student's ID number on it), VISA, or MASTERCARD. Please complete and return your student's yellow registration form even if you are unable to include payment at this time. 

3. Review the classes listed on the bottom of the Fee Schedule page for the courses you have been assigned. All efforts have been may to ensure that your schedule contains the classes you requested last spring for this school year. In some cases, original requests have been replaced with alternative selections due to course availability restrictions or conflicts in the student's schedule. For further information such as when request for schedule changes can be made, please refer to your registration packet.

8. Bus Information: Students who live outside a radius of 1.5 miles and live within the designated attendance area are granted bus privileges to and from school. If you are anticipating bus privileges, please refer to your registration packet.  

4. For information regarding Student Internet Access, District and News Media Coverage, and Student Handbook and Statement of Awareness Cards, please refer to your registration packet.

9. Identification (ID) Card pictures: Please refer to your registration packet.

5. Absence Reporting Policy: Please read the Absence Reporting Policy located in your registration packet. PARENT/GUARDIAN SIGNATURE IS REQUIRED ON THE FORM.

 


If for some reason you miss the mail-in registration deadline
bring your completed forms to school and place in the drop box located in the front office
PLEASE DO NOT WAIT UNTIL THE FIRST DAY OF SCHOOL
