Flu Shots
Flu shot clinics are being scheduled at the following sites in the district again this year. If you are covered by UnitedHealthcare through Mesa Public Schools, the flu shot is FREE. Just present
your ID card.

For all others, the cost is $20. They will accept cash or checks. Checks should be payable to Scottsdale Healthcare and is due upon receiving the shot.

Mobile On Site Mammography (M.O.M)
Please contact M.O.M. at: (480) 967-3767 if you have any questions or to schedule your convenient appointment at one of the locations listed below. Please have your insurance card available.

Cost: Insurance Coverage with UnitedHealthcare and other select carriers. Non-Insured patients pay $138.00 (includes the Radiologist Reading). In addition to the regular interpretation by the Radiologist, we are pleased to offer the R-2 Computerized reading for an additional $28.00. (Covered by UnitedHealthcare).

Please bring the following with you on the day of your appointment:
Your insurance card, your Dr’s name and address, and the location of where you had your prior mammogram films.

MOM is unable to accommodate implant patients except at our Tempe Office location.
It is the patient’s responsibility to consult their individual plan for coverage and limitations. You will be responsible for any charges that are not covered. Patients under 35 must provide a Dr’s referral. Baseline mammograms start at age 35-39, and most insurance plans cover women every year over 40 year’s old.
