Medications: All medications are kept in the Health Office, in their original container labeled for that student. A consent form needs to be completed by parents and physicians, and will be given as directed.
Immunizations: Arizona law requires children to be up to date on their immunizations to attend school. Any questions, please call the Health Office.
Screenings: The health office does vision screenings for all Pre-K, Kinder, 1st, 3rd, 5th grade, new students, and Special Ed. students. We also do hearing screenings for all Pre-K, Kinder, 1st, 2nd and 6th grades, new students and Special Ed. students. We would be happy to screen any students upon request.
Special Dietary needs: Any student who has dietary needs has to complete a form from the Food and Nutrition Department. There are 2 parts to the form, one for the parents to fill out, and one for the physician to fill out. The Health Office has these forms for you.
Chronic Health Conditions: If your child has a serious chronic health conditions, such as anaphylaxis (severe allergy to a substance where an Epi Pen is needed at school for a life-threatening allergic reaction), asthma, diabetes, or seizures, please inform the Health Office. We will work with you in putting a health care plan in place for the safety of your child while they are in school. As part of the process we will make sure that all of their teachers are aware of the condition.
