Special Dietary Accomodations

Mesa Public Schools Food and Nutrition Department has registered dietitians on staff to assist students with dietary accommodations for medical conditions.

Medical Statement for Children with Special Dietary Accommodations

Requests for meal accommodations must be supported by a statement explaining the requested food substitution and must be signed by a recognized medical authority. The Medical Statement must include:

  An explanation of how exposure to the food affects the child.

  The food(s) to be omitted from the child’s diet

  The food(s) to be substituted

Items specifically listed on medical referral forms by a recognized medical authority as an allergen or intolerance will be noted in the students record and meal trays will be monitored so not to include identified food allergens.  Due to the possibility of residual or cross contact of allergens and non-allergens in manufacturing plants and the ability of manufacturers to omit small percentage ingredients in ingredient listings it is impossible to guarantee an allergen free environment.

The form that must be completed and submitted to the Food and Nutrition Department for a special dietary needs request is available on this web page, through the school's health office and from the cafeteria manager.
