Registration of new students to Mesa Schools will require the following:

  • Official birth certificate
  • Record of up-to-date immunizations required by AZ State Law
  • Proof of residency
  • Withdrawal form from previous school

Students transferring in from another Mesa Elementary School will need to complete a registration and emergency information form and provide proof of residency within the Lowell School boundaries prior to assignment to a classroom.

IMMUNIZATIONS REQUIRED 
 
DTaP/DTP (Diptheria, Tetanus, Pertussis) - For Pre-school, K and 1st grade, four doses are required if at least one dose was given on or after the 4th birthday; if 5 doses were given before the 4th birthday, a 6th dose is required.  For 2nd through 12th grades, four doses are required if at least one dose was given on or after the 4th birthday; a 5th dose is required if the 4th dose was given before the 4th birthday.
 
Polio (OPV, IPV) - For Pre-school through 12th grade, three doses are required; a 4th dose is required if the 3rd dose was given before the 4th birthday (4 doses appropriately spaced are complete, regardless of age at last dose)
 
MMR (Measles, Mumps, Rubella) - For Pre-school, one dose is required on or after the first birthday; if the 1st dose was received before the first birthday, a second dose is required.  For K and older, 2 doses are required, the 1st dose on or after the first birthday with the 2nd dose a minimum of 28 days later.
 
HIB (Haemophilus Influenza type b) - For Pre-school only, a minimum of 2 doses are required, unless the 1st dose was given on or after 15 months (then only 1 dose required); this vaccination is not required after the age of five.
 
Hepatitis A - For Pre-school only, 2 doses, six months apart, are required.
 
Hepatitis B - For Pre-school through 12th grade, 3 doses are required, 3rd dose must be given after 24 weeks of age.
 
Meningococcal (MCV4) - One dose required only for 6th and 7th graders 
 
Tdap (Tetanus, Diptheria, Pertussis) - Required only for 6th and 7th graders , if 11 years or older and it has been 5 years since the last dose of DTP, DTaP, DT or Td.
 
Varicella - Required for Pre-school through 5th grade,  1 dose on or after the first birthday OR a history of the disease (parents/guardian can report history).
