This Step by Step Process form is used to guide the process from teacher concern to the MTSS Intervention Process
The Early Documentation Action Items form is used as a checklist to ensure teacher communication with parents and to rule out disadvantage factors such as hearing, vision and attendance before proceeding with interventions.
The Collaborative Team Student Intervention Plan is used when a grade level/PLC team discusses student data, plans and monitors initial interventions.
The MTSS Team Documentation form is used when the Collaborative Team interventions have been unsuccessful and more targeted interventions are needed.
The MTSS Initial Meeting Agenda is to guide documentation of the student concerns, what intervention(s) are appropriate, and duration. This meeting involves parents.
The MTSS Intervention Data Collection Sheet is used to discuss the details of the goal and intervention. It allows the team to enter the progress of the student and verify fidelity of intervention.
This form is used to interview parents when there is a concern about an ELL student and the team needs further information.
